Each era of political structure brings, in its own way, something that our nation needed at that specific point in time. With each need answered comes a needed question, for that which works for one era make have disastrous consequences for the next. Whereas the founding era, or the eighteenth century to the american revolution, questioned the constitution, the time after questioned the union and Lincoln’s words. The questions asked during each era were crucial to the development of our nation and government. Without this chain of events and the specific form of thinking for each era, we would not be where we are today. Perhaps the most important era is the one that continues to this day. The progressives are openly critical of the Declaration of Independence and as a majority, they disagreed and even distrusted the foundation of the American founding. As Wilson was quoted saying in 1911, â€Å"If you want to understand the Declaration of Independence, do not repeat the Preface†. By taking out the preface of the Declaration of Independence, the only part left is the initial complaints agains the Monarch of England. The whole section dictating how to form a government is no longer available. Progressives like Wilson, felt that the only thing the Founders were qualified to put down in writing that would resist the test of time, was their displeasure with England, not how to run a new country. At the time of the writing of the Declaration, the main problem was getting rid of tyranny. Specifically, within the Declaration of Independence, Jefferson explicitly states that the document is â€Å"universal, absolute truth†, meaning that every law set to govern the nation will hold true forever. Cause and Effects of Smallpox Smallpox is caused by the variola virus that emerged in human populations thousands of years ago. Smallpox is a specific, infectious, and highly contagious febrile disease known only to be transmitted by humans. It is caused by a virus from air currents which are eventually passed on from person to person. Smallpox varies from a mild form without skin manifestations to a highly fatal hemorrhagic form. Edward Jenner, an English physician, discovered a means of preventing smallpox through vaccination. Gradually mass vaccination programs were introduced in many parts of the world. Smallpox was the first disease conquered by human beings and was eradicated by vaccination. After the incubation period, high fever, chills, severe headache and backache, and general malaise begin to develop. Your body temperature can reach up to 106 degrees Fahrenheit. After 2 to 4 days, a rash emerges first as small red spots on the tongue and in the mouth. The spots develop into sores which spread large amounts of the virus into the mouth and throat. The person becomes most contagious during this phase. Soon after, the smallpox rash appears on exposed portions of the body: the face, forearms, wrists, palms, lower legs, feet, and soles. Usually the rash spreads to all parts of the body within 24 hours. As the rash appears, the fever usually falls and the person may start to feel better. Question: Discuss about the Changes in Blood Pressure Classification. Answer: Introduction: Hypertension is the most prevalent disease in Australia. Hypertension mainly occurs due to irregular food habit and unhealthy lifestyle. Hypertension can be classified into different categories based on the level of blood pressure. There are varied complications and different treatment options are associated with each category of hypertension. In this essay case of Amanda and Jim is discussed. Amanda and Jim belong to different grades of hypertension. Hence, different goals should be set and different treatment should be given to Amanda and Jim. Educational plan for control of hypertension in case of Amanda and Jim is also discussed. This educational plan should comprise of education for food, physical exercise, smoking and alcohol consumption, psychological and social issues, blood pressure monitoring and medication. Prevalence of hypertension: High blood pressure ( 140/90 mmHg) is the most frequently treated disease in Australia. In Australia, approximately 9 out of 100 patients are hypertensive patients treated at general practitioner. Majority of these patients are with chromic hypertension and only 4.5 % patients are newly diagnosed patients. Prevalence of hypertension is slightly higher in men (34 %) as compared to the women (29 %). 50 % patients of hypertension are generally associated with other heart and cardiovascular complications and approximately 11 % are associated with diabetes. In Australia, it is observed that high blood pressure increases with the age of the person (ABS, 2013). Primary hypertension can be mainly caused due to the smoking, being overweight or obese, alcohol consumption, lack of exercise, unhealthy diet comprising of high salt and saturated fat and family history of high blood pressure. Secondary hypertension can be mainly caused due to kidney disease, endocrine disease and narrowing of aorta. Secondary hypertension can also be caused due to steroidal drugs, contraceptive pill and pregnancy (Harrison et al., 2017). Grading system for hypertension: Blood pressure can be graded in three categories like grade 1 (SBP 140-159 or DBP 90-99), grade 2 (SBP 160-179 7or DBP 100-109) and grade 3 (SBP 180 or DBP 110). Grade 1 hypertension mainly occurs due to circulatory, vascular or renal response to environmental and genetic stimuli. In grade 1 hypertension, there are structural and functional alterations in heart and arteries of the patient. There can be occurrence of more than one risk factors in grade 1 hypertension patients. There would not be target organ damage in patients with grade 1 hypertension. Amanda may be suffering through grade 1 hypertension. Grade 1 hypertension patients should be assessed to identify early stage cardiovascular marker in the patient and identify specific treatment to attenuate these markers (Trana and Giangb, 2014; Kim et al., 2017). Patients of grade 1 hypertension should be traded with antihypertensive drugs like angiotensin converting enzyme inhibitors (ACE inhibitors) or angiotensin II receptor an tagonists. For Amanda also same drug was prescribed. Grade 2 hypertension is considered as the progressive disease. Grade 2 hypertension can occur due to inability of heart and cardiovascular system to control blood pressure. This may occur due to the persistent structural and functional changes in the heart and arteries. Grade 2 hypertension can be associated with the more than two risk factors. If grade 2 hypertension doesnt controlled before certain level it may lead to the target organ damage. This susceptibility of the target organ damage in grade 2 patients should be identified through research studies. This would be helpful to manage the condition effectively and in cost effective manner. Jim might have grade 2 hypertension, because he was diagnosed with type 2 diabetes mellitus and atherosclerosis. In grade 2 hypertensive patients, there is moderate to high rise in blood pressure accompanied by at least two target organ damage. Hence in these patients antihypertensive drugs like ACE inhibitors or angiotensin II receptor blocker s or beta blockers or calcium channel blockers or renin inhibitors can be used (Varounis et a., 2017; Yoon et al., 2015). Along with antihypertensive drugs, there should be administration of medication for target organ pathology. In patients with hypertension associated with atherosclerosis, there should be administration of statins which lowers levels of cholesterol. For Jim also, Lipitor was prescribed. Moreover, there was clear indication of organ damage in Jim. Patients with grade 3 hypertension are apparently associated with the target organ damage. Patients of this category generally exhibit sustained cardiovascular events irrespective of the level of blood pressure. Aging and association of other risk factors generally exaggerate condition of the grade 3 hypertensive patients and it may lead to the morbidity and mortality. Special emphasis should be given for reducing cardiovascular risk factors in grade 3 hypertensive patients. In patients with grade 3 hypertension, emergency antihypertensive drugs like nitrates should be administered along with the other antihypertensive drugs. In addition, medications for other risk factors should be administered in patients with grade 3 hypertension. Modifiable features of hypertension comprises of smoking, physical activity, low density lipoprotein (LDL), triglyceride levels, obesity, body mass index (BMI), diabetes mellitus and psychological stress. Non-modifibale features of hypertension comprises of family history, age, gender and genetic factors (Materson et al., 2014). Nurse should set different goals for Amanda and Jim. Nurse should set goal for Amanda to reduce her blood pressure to SBP 130-149 or DBP 80-89. Nurse should set goal for Jim to reduce his blood pressure to SBP 140-159 7or DBP 90-99. Nurse should set goal for Jim to reduce his cardiovascular complications and manage his type 2 diabetes mellitus condition. Nurse should give special attention for diastolic pressure in case of Jim because there should not be more reduction in diastolic pressure in patients with diabetes. Educational Plan: Food Nurse should educate Amanda and Jim for dietary habit to control hypertension. Nurse should advise them to eat food comprising of fruits, vegetable, whole grains, low-fat dairy products and fish. Nurse should advise them to consume more amount of potassium and fewer amounts of saturated fats. Specifically Jim, should avoid sugar containing products and food containing saturated fatty acids because he was diagnosed with type 2 diabetes mellitus and atherosclerosis. Nurse should prepare diet plan for Amanda and Jim, in consultation with dietician. These products exaggerate blood pressure in hypertension patients. Nurse should advise them to consume not more than 2000 mg per day salt because salt has negative impact on hypertension and its related risks like diabetes and kidney disease (Chen et al., 2014, Khan et al., 2014). Physical exercise Nurse should advise them to keep control on their body weight. Due to increase in body weight, BMI also can increase. Increased BMI is one of the prominent risk factor for the hypertension. Amanda should give special attention to control her body weight because her BMI is 28. BMI in this range indicates, Amanda is overweight. For maintaining optimum body weight both Amanda and Jim should increase their physical activity. Amanda should be very particular about physical activity because she should reduce her body weight. Amanda and Jim should perform approximately 150 minutes aerobic activity per week. Physical activity on the regular basis would be helpful for Amanda and Jim for controlling hypertension, managing stress due to increased blood pressure, reducing risks associated with hypertension and reducing body weight. Nurse should prepare exercise schedule for Amanda and Jim in consultation with exercise physiologist (Lauziere et al., 2013; Zinat Motlagh et al., 2015). Smoking and alcohol consumption Nurse should advise them to avoid alcohol because alcohol can increase blood pressure in normal persons also. Nurse should advise them to avoid smoking because smoking can produce injuries to blood vessels and facilitate hardening of the blood vessels (Lauziere et al., 2013; ; Zinat Motlagh et al., 2015) Psychological education Nurse should advise them to live stress free life. They should practice muscle relaxation and deep breathing for living stress free life. Physical activity and sufficient sleep also would be helpful in reducing stress (Lauziere et al., 2013; ; Zinat Motlagh et al., 2015) Blood pressure monitoring Nurse should teach them to monitor blood pressure at home. She should educate them to check blood pressure prior to and after medications. Nurse should incorporate cardiovascular technologist for teaching them measurement of blood pressure. This would be helpful in assessing effectiveness of medications. Nurse should also advise them to consult doctor immediately, if they observed noteworthy variation in their blood pressure (Lauziere et al., 2013) Social education Both Amanda and Jim should link themselves to the different community healthcare societies. In these societies they can share their experience and gain knowledge about the disease. It would be helpful for them for self management of the disease. It would also be helpful in relieving psychological issues like stress and burden of the disease. They should consult social worker to get link themselves to community healthcare societies (Park et al., 2011). Education about Lipitor and Avapro Nurse should educate Jim for Lipitor consumption. Nurse should advise him to take this drug once daily at the same time every day. Nurse should advise him to take regular blood tests for cholesterol and triglycerides. Nurse should also advise him to take liver function test. Nurse should advise him to inform healthcare provider about Lipitor consumption, if doctor wishes to prescribe him medicines for acute illness. Nurse should inform him that there may be nausea, vomiting and joint pain due to consumption of this medication. Nurse should tell him that, he should report muscle pain, weakness, fever, swelling and change in color of urine during consumption of Lipitor (Barber et al., 2012). Nurse should educate Amanda that she should not consume Avapro, if she has diabetes. Nurse should tell her that she should not use Avapro, if she is dehydrated. Nurse should inform her that she should monitor blood pressure on regular basis during consumption of Avapro. If she didnt find improvemen t in blood pressure, she should consult doctor. She should not stop taking Avapro, even if she found improvement in her blood pressure. She should keep Avapro away from heat and light. Nurse should tell her that she should stop alcohol and potassium supplements while taking Avapro. Both Amanda and Jim should consult pharmacist to get more information about their respective drugs (Barber et al., 2012). Conclusion: In Australia, highest number of treatments at general practitioners is for hypertension. Male are more affected with hypertension as compared to the female. There are different treatment options are available for different grades of hypertension. Along with medications use change in food habit and lifestyle should be incorporated in the management of hypertension. Amanda and Jim are having different grade of hypertension. There should be different goals and education plan for Amanda and Jim in hypertension management. Comparative Genomics Research Benefits ABSTRACT The rapidly emerging field of comparative genomics has yielded dramatic results. Comparative genome analysis has become feasible with the availability of a number of completely sequenced genomes. Comparison of complete genomes between organisms allow for global views on genome evolution and the availability of many completely sequenced genomes increases the predictive power in deciphering the hidden information in genome design, function and evolution. Thus, comparison of human genes with genes from other genomes in a genomic landscape could help assign novel functions for un-annotated genes. Here, we discuss the recently used techniques for comparative genomics and their derived inferences in genome biology. INTRODUCTION Comparative genomics is the study of the relationship of genome structure and function across different biological species or strains. Comparative genomics is an attempt to take advantage of the information provided by the signatures of selection to understand the function and evolutionary processes that act on genomes. While it is still a young field, it holds great promise to yield insights into many aspects of the evolution of modern species. The sheer amount of information contained in modern genomes (750 megabytes in the case of humans) necessitates that the methods of comparative genomics are automated. Gene finding is an important application of comparative genomics, as is discovery of new, non-coding functional elements of the genome. Human FOXP2 gene and evolutionary conservation is shown in a multiple alignment (at bottom of figure) in this image from the UCSC Genome Browser. Note that conservation tends to cluster around coding regions (exons). Comparative genomics exploits both similarities and differences in the proteins, RNA, and regulatory regions of different organisms to infer how selection has acted upon these elements. Those elements that are responsible for similarities between different species should be conserved through time (stabilizing selection), while those elements responsible for differences among species should be divergent (positive selection). Finally, those elements that are unimportant to the evolutionary success of the organism will be unconserved (selection is neutral). SCOPE OF COMPARATIVE GENOMICS One of the important goals of the field is the identification of the mechanisms of eukaryotic genome evolution. It is however often complicated by the multiplicity of events that have taken place throughout the history of individual lineages, leaving only distorted and superimposed traces in the genome of each living organism. For this reason comparative genomics studies of small model organisms (for example yeast) are of great importance to advance our understanding of general mechanisms of evolution. Having come a long way from its initial use of finding functional proteins, comparative genomics is now concentrating on finding regulatory regions and siRNA molecules. Recently, it has been discovered that distantly related species often share long conserved stretches of DNA that do not appear to code for any protein. One such ultra-conserved region, that was stable from chicken to chimp has undergone a sudden burst of change in the human lineage, and is found to be active in the developing brain of the human embryo. Computational approaches to genome comparison have recently become a common research topic in computer science. A public collection of case studies and demonstrations is growing, ranging from whole genome comparisons to gene expression analysis. This has increased the introduction of different ideas, including concepts from systems and control, information theory, strings analysis and data mining. It is anticipated that computational approaches will become and remain a standard topic for research and teaching, while multiple courses will begin training students to be fluent in both topic. Chromosomes from two genomes are drawn: human chromosome 1 (drawn with a global zoom factor of 50x) and mouse chromosomes 1-19, X, and Y with mouse chromosome 3 drawn enlarged 10-fold. The mapping of regions across two genomes can be one-to-one in absence of duplication events; one-tomany if a region has undergone duplication or loss in one of the species, or many-to-many if duplication/loss has occurred in both lineages. Fitch et al., developed a method called BBH (Best Bidirectional Hits), which identifies gene pairs that are best matches of each other as orthologous. Tatusov et al., further enhanced this method, which matches groups of genes to groups of genes. Understanding the ancestry of the functional elements compared is central to our understanding and applications of genome comparison. Most comparative methods have focused on one-to-one orthologous regions, but it is equally important to recognize which segments have undergone duplication events, and which segments were lost since the divergence of the species. Comparing segments that arose before the divergence of the species may result in the wrong interpretations of sequence conservation and divergence. Further, in the presence of gene duplication, some of the evolutionary constraints that a region is under are relieved, and uniform models of evolution no longer capture the underlying selection for these sites. Thus, our methods for determining gene correspondence should account for duplication and loss events, and ensure that the segments we compare are orthologous Applications Gene identification Once genome correspondence is established, comparative genomics can aid gene identification. Comparative genomics can recognize real genes based on their patterns of nucleotide conservation across evolutionary time. With the availability of genome-wide alignments across the genomes compared, the different ways by which sequences change in known genes and in intergenic regions can be analyzed. The alignments of known genes will reveal the conservation of the reading frame of protein translation. The genome of a species encodes genes and other functional elements, interspersed with non-functional nucleotides in a single uninterrupted string of DNA. Recognizing protein-coding genes typically relies on finding stretches of nucleotides free of stop codons (called Open Reading Frames, or ORFs) that are too long to have likely occurred by chance. Since stop codons occur at a frequency of roughly 1 in 20 in random sequence, ORFs of at least 60 amino acids will occur frequently by chance (5% under a simple Poisson model), and even ORFs of 150 amino acids will appear by chance in a large genome (0.05%). This poses a huge challenge for higher eukaryotes in which genes are typically broken into many, small exons (on average 125 nucleotides long for internal exons) in mammals. The basic problem is distinguishing real genes those ORFs encoding a translated protein product from spurious ORFs the remaining ORFs whose presence is simply due to chance. In mammalian genomes, estimates of h ypothetical genes have ranged from 28,000 to more than 120,000 genes. Applications of comparative genomics to wheat A number of important major traits requiring elucidation in wheat are essentially non-polymorphic. Thus there is no prospect of creating a mapping population which is the starting point of all positional cloning strategies in most species to date. Moreover given the size of the wheat genome, many traits lie in regions where the gene density per BAC is one or two, making it difficult if not impossible to walk from one wheat BAC to the next. The Ph1 locus (controlling chromosome pairing in wheat) is one such example, in which the starting point was wild type wheat and a mutant carrying a deletion of more than 70Mb (almost the size of the whole Arabidopsis genome). Its phenotype is not easy to score. My group wished to characterise this locus. We created three different types of mutagenised populations, sequenced the equivalent rice Ph1 region, built BAC libraries (all are now available free of IP) for Brachypodium (a small genome species more closely related to wheat), sequenced Brachy podium Ph1 equivalent region, built a hexaploid (CS) (737,000 clones) wheat in collaboration with INRA (providing a further 500,000 clones), exploited Jorge Dubcovskys Tetraploid wheat BAC library, sequenced wheat BACs and defined the tissues in which the Ph1 phenotype is expressed. I will discuss the approaches adopted and resources created. The most significant application of comparative genomics in molecular medicine is the identification of drug targets of many infectious diseases. For example, comparative analyses of fungal genomes have led to the identification of many putative targets for novel antifungal. This discovery can aid in target based drug design to cure fungal diseases in human. Comparative analysis of genomes of individuals with genetic disease against healthy individuals may reveal clues of eliminating that disease. Comparative genomics helps in selecting model organisms. A model system is a simple, idealized system that can be accessible and easily manipulated. For example, a comparison of the fruit fly genome with the human genome discovered that about 60 percent of genes are conserved between fly and human. Researchers have found that two-thirds of human genes known to be involved in cancer have counterparts in the fruit fly. Even more surprisingly, when scientists inserted a human gene associated with early-onset Parkinsons disease into fruit flies, they displayed symptoms similar to those seen in humans with the disorder, raising the possibility that the tiny insects could serve as a new model for testing therapies aimed at Parkinsons. Thus, comparative genomics may provide gene functional annotation. Gene finding is an important application of comparative genomics. Comparative genomics identify Synteny (genes present in the same order in the genomes) and hence reveal gene clusters. Comparative genomics also helps in the clustering of regulatory sites, which can help in the recognition of unknown regulatory regions in other genomes. The metabolic pathway regulation can also be recognized by means of comparative genomics of a species. Dmitry and colleagues have identified the regulons of methionine metabolism in gram-positive bacteria using comparative genomics analysis. Similarly Kai Tan and colleagues have identified regulatory networks of H. influenzae by comparing its genome with that of E. coli. The adaptive properties of organisms like evolution of sex, gene silencing can also be correlated to genome sequence by comparative genomics. CONCLUSION The most unexpected finding in comparing the mouse and human genomes lies in the similarities between junk DNA, mostly retro-transposons, (transposons copied from mRNA by reverse transcriptase) in the two species. A survey of the location of retrotransposon DNA in both species shows that it has independently ended up in comparable regions of the genome. Thus junk DNA may have more of a function than was previously assumed. High performance computing tools help in comparing huge genomes. Because of its wide applications and feasibility, automation of comparing genomics is possible. Such Comparisons can aid in predicting the function of numerous hypothetical proteins. Cloning is Ethically and Morally Wrong The question shakes us all to our very souls. For humans to consider the cloning of one another forces them all to question the very concepts of right and wrong. The cloning of any species, whether they be human or non-human, is ethically and morally wrong. Scientists and ethicists alike have debated the implications of human and non-human cloning extensively since 1997 when scientists at the Roslin Institute in Scotland produced Dolly. No direct conclusions have been drawn, but compelling arguments state that cloning of both human and non-human species results in harmful physical and psychological effects on both groups. The following issues dealing with cloning and its ethical and moral implications will be addressed: cloning of human beings would result in severe psychological effects in the cloned child, and that the cloning of non-human species subjects them to unethical or moral treatment for human needs. The possible physical damage that could be done if human cloning became a reality is obvious when one looks at the sheer loss of life that occurred before the birth of Dolly. Less than ten percent of the initial transfers survive to be healthy creatures. There were 277 trial implants of nuclei. Nineteen of those 277 were deemed healthy while the others were discarded. Five of those nineteen survived, but four of them died within ten days of birth of sever abnormalities. Dolly was the only one to survive (Fact: Adler 1996). If those nuclei were human, "the cellular body count would look like sheer carnage" (Logic: Kluger 1997). Even Ian Wilmut, one of the scientists accredited with the cloning phenomenon at the Roslin Institute agrees, "the more you interfere with reproduction, the more danger there is of things going wrong" (Expert Opinion). The psychological effects of cloning are less obvious, but none the less, very plausible. In addition to physical harms, there! are worries abou t the psychological harms on cloned human children. One of those harms is the loss of identity, or sense of uniqueness and individuality. Many argue that cloning crates serious issues of identity and individuality and forces humans to consider the definition of self. Gilbert Meilaender commented on the importance of genetic uniqueness not only to the child but to the parent as well when he appeared before the National Bioethics Advisory Commission on March 13, 1997. He states that "children begin with a kind of genetic independence of [the parent].
